Please check with your insurance company to determine if your plan includes a copay, deductible, and/or co-insurance. If you have a copay, it is due at the time of service. Deductible and co-insurance balances, if applicable, will be billed to the patient following receipt of the Explanation of Benefits (EOB) from the insurance company.
